There's a few things you'll need to know before you come in for your appointment.

• You need a referral form.

• You will also need to fill out a basic patient info sheet once you arrive.

• If your doctor has checked off the patient pay box you will need to pay for services when you arrive before the appointment proceeds. 

• We will provide you with a statement that gives you a breakdown of the services provided. You can submit this form to your dental insurance. If the images are covered by your insurance,  your insurance company will reimburse you directly. 

• We will then begin the procedure your doctor specified. 

• Once the appointment is complete, we will have the finished images sent to your doctor within 7-10 business days.
